Cardiovascular Risk Management: An aggressive approach to saving lives.

Since 1900, cardiovascular disease (CVD) has been the number one killer in the U.S. every year but 1918. It claims almost as many lives each year as the next five leading causes of death combined, which are cancer, chronic lower respiratory diseases, accidents, diabetes mellitus, influenza and pneumonia (from the American Heart Association, Heart Disease and Stroke Statistics, 2005 update).

When Health By Design discovers patients to be at risk for CVD in its Wellness Assessment Program, we offer intensive preventive and wellness strategies to avert cardiovascular disease. Our program includes lunch-hour lectures, written education materials and monthly doctor visits over a six-month period. Goals of the program include:

  • Assessing a patient’s risk for cardiovascular disease
  • Recommending any further work-up that may be necessary to evaluate risk
  • Developing a treatment plan to address risk
  • Educating patients on cardiac disease and risk management
  • Following-up to monitor improvement
