For many years, on-site wellness assessments have been the centerpiece and starting point of Health by Design’s products and services. Armed with extensive laboratory and health history information, our doctors and nurses provide employees and their family members with the tools to better understand their current health and assist with improving their quality of life.

Wellness assessments save lives and boost a company's bottom line. Averting a health care catastrophe is a wise investment compared to the cost of treating a health catastrophe after it occurs. It's the smart approach to good health.

Health by Design’s Wellness Assessment program is comprised of the following:

• Extensive laboratory screenings; height, weight and blood pressure readings
• Health Risk Assessment and comprehensive customized report
• Face-to-face nurse counseling or coaching by telephone with physician oversight
• Individualized health improvement action plan
• Referrals to HBD on-site clinic and lifestyle managers and community health resources
